Who we are
This website is operated by Student Evaluation Center, LLC ("we", "us"), 110 E Market St. (The Root Building), New Albany, IN 47150, United States. We are the data controller for the information described on this page. Contact: drterryburger@gmail.com, 812-946-6411.
TODO(client): confirm the legal entity name and the address to be used for privacy correspondence.
What this website collects
The website itself sets no cookies and uses no analytics or tracking scripts. The only information we collect through this website is what you choose to send us:
- Contact form: your name, your email address and the message you write. Nothing else is collected by the form.
- Server logs: our hosting provider records standard technical data (IP address, browser type, pages requested, time of request) needed to deliver the site securely.
The website does not use Google Analytics, Facebook Pixel or any similar tracker. If that changes, this policy will be updated first.
Why we use it and on what basis
- To reply to your enquiry and, if you ask us to, to arrange a consultation. Basis: your request (steps taken at your request before entering an agreement) and our legitimate interest in responding to enquiries.
- To keep the website secure and available. Basis: our legitimate interest in running the site.
Health information is not requested by the contact form. If you include health details about yourself or a child in your message, we treat them as confidential and use them only to respond to you. Clinical records for clients of the practice are handled under the practice's separate clinical privacy practices, not this website policy.
Who processes your information
- Netlify, Inc. (United States) hosts the website and receives contact-form submissions, which are stored in our Netlify account and emailed to us.
- Calendly, LLC (United States) handles consultation bookings. Calendly is a separate website with its own privacy policy; nothing is sent to Calendly until you leave this site and book there.
- Google Maps (Google LLC, United States) provides the embedded map on the consultation and contact section. Loading the map sends your IP address and browser details to Google and may set Google cookies, subject to Google’s privacy policy. The map is lazy-loaded, so it is only requested once you scroll to it.
- Facebook is linked from the footer; no Facebook code runs on this site.
How long we keep it
Contact-form submissions are kept in our Netlify account and in our email until they are no longer needed to respond to you, and then deleted. TODO(client): confirm the retention period for enquiries (for example, 12 months after the last contact) and who deletes them.
Your rights
You can ask us what information we hold about you, ask us to correct or delete it, or object to our use of it. Email drterryburger@gmail.com and we will respond as soon as possible. Residents of certain jurisdictions may have additional rights under local law. TODO(client): confirm whether any state privacy law (for example Indiana's Consumer Data Protection Act thresholds) applies to the practice.
Children
This website is intended for parents, guardians, adults seeking evaluation and school professionals. We do not knowingly collect information from children through the website.
Changes
If we add any tool that collects information (for example analytics or an embedded booking widget), this policy will be updated before it goes live.
